Exposure and Response Prevention (ERP) for OCD

OCD does not always look like the version shown on television. It can be quiet, exhausting, and easy to hide, built from intrusive thoughts, mental rituals, or checking behaviours that take up far more of your day than anyone around you realizes. Exposure and Response Prevention, or ERP, is the therapy approach with the strongest evidence base for OCD, and it is the primary method our therapists use to treat it. Healing In Therapy offers ERP for OCD across Nova Scotia and New Brunswick through in-person and secure virtual sessions.

What Is ERP for OCD?

ERP is a structured form of cognitive behavioural therapy built specifically for obsessive-compulsive disorder. It works on a simple but demanding principle: you gradually face the thoughts, images, situations, or sensations that trigger your obsessions, while resisting the urge to perform the compulsion that normally follows.

Compulsions, whether visible ones like handwashing or checking, or mental ones like reviewing, counting, or seeking reassurance, work in the short term. They lower anxiety for a few minutes. Over time they do the opposite of helping. ERP interrupts that cycle by teaching your nervous system, through direct experience, that the anxiety will rise and fall on its own without the ritual.

How ERP Therapy Works

Your therapist starts by mapping your specific obsessions and compulsions in detail, since OCD rarely looks the same from one person to the next. From there, you and your therapist build a hierarchy, a ranked list of feared situations from mildly uncomfortable to highly distressing. Exposures begin at a level you can tolerate and move upward at a pace you help set.

Sessions typically run 50 to 60 minutes. Many clients see meaningful movement within 12 to 20 sessions, though OCD severity and the number of obsession themes involved affect that timeline. We offer daytime, evening, and weekend appointments, with secure video sessions available anywhere in Canada.

OCD Presentations ERP Can Help With

- Contamination fears and washing or cleaning rituals

- Checking behaviours, such as locks, appliances, or messages sent

- Intrusive thoughts of a violent, taboo, or unwanted sexual nature

- Relationship-focused OCD, including constant doubt about a partner

- Moral or religious scrupulosity

- Symmetry, order, or "just right" compulsions

- Mental compulsions such as reviewing, counting, or silent reassurance-seeking

- Health anxiety tied to obsessive symptom-checking

If you are unsure whether what you are experiencing is OCD or general anxiety, that distinction is something your therapist can help clarify in an initial session.

What Changes with ERP

- Less time lost to rituals: Compulsions that once took hours shrink or disappear, freeing up your day.

- Reduced anxiety around triggers: Situations that used to feel dangerous start to feel neutral.

- More trust in your own judgment: You rely less on rituals or reassurance to feel certain.

- Greater flexibility: Avoidance patterns that had narrowed your life start to loosen.

- A different relationship with intrusive thoughts: You learn to let a thought pass through without needing to respond to it.

Why Choose ERP at Healing In Therapy

Our therapists working with OCD clients across Nova Scotia, New Brunswick, and Canada are trained in ERP protocols and understand how OCD can present differently across contamination, checking, intrusive thought, and relationship-focused themes. We also recognize that OCD often carries shame that keeps people from seeking help sooner. Sessions are structured to be direct and practical without judgment about the content of your obsessions.
